Admissions

Gould Farm is a private, not for profit corporation under IRS Code 501 (c) (3) receiving no government funding or insurance payments. We serve without regard to race, creed or national origin:

Admission Criteria

  • adults 18 and older
  •  with major mental illness: depression, bipolar disorder, schizoaffective disorder, schizophrenia and related disorders at a sub-acute level
  •  with co-occurring disorders at a non acute level or fully in remission.
  •  who are voluntarily in treatment and are medication compliant
  • who can contract for safety and acceptable behavior within an open, normative residential setting with a 30 hour/wk structure of work, community life, and non-intensive clinical supports
  •  and who are supported financially and logistically by a private family partnership.

Gould Farm cannot serve those with: a history of arson, sexual offense, characterological or sociopathic disorders, violence, unresolved legal entanglements, mental retardation, severe developmental disorders, active substance disorders, a refractory constellation of disorders, complications including but not limited to pregnancy, or those wholly dependent upon state-supported services.

Requirements for admission:

  • A referral from a treating psychiatrist (see attached referral form) with current, signed medication orders and a two-week supply of medication in hand
  • Copies of recent hospital and treatment records
  • A signed Private Partnership Agreement for appropriate payment of fees and plan for living accommodations and appropriate continuation of care if applicant chooses not to remain in program or is requested to leave the program.
  • An in-person or phone interview with applicant
  • A two-week evaluation visit with the expectation of full acceptance into program at the end of this evaluation period. Gould Farm reserves the right to deny acceptance, if, for any reason, we do not see our services to be an appropriate match for a given individual at a given time.
  • Gould Farm requests, when possible, a biographical narrative from both the family and the applicant, and completion of a General Health Assessment.
  • Applicant s may be required to submit evidence of abstinence from drugs and alcohol for a specific period of time.

On intake and during the two-week evaluation period applicants will be required to sign Gould Farm’s

Residency Agreement, Communication Agreement, Budgeting Treatment Plan, Consent for Prescription of Medication, Consent for Routine and Preventative Care, and the Admissions Data Form.

All applicants will be given access to a Notice of Privacy Practices and sign acknowledgement of such.

All applicants will be oriented to and sign acceptance of all provisions of the Guest Agreement and general community guidelines as outlined in the Guest Handbook.
